Hold Harmless Consent & Release form

I, the undersigned, hereby acknowledge that I am voluntarily participating in the driver coaching event hosted by [Event Organizer/Company Name] on [Date] at [Location]. I understand that this event involves inherent risks and dangers that may result in physical injury, property damage, or death.

In consideration of being allowed to participate in this event, I agree to the following:

1. Acknowledgment of Risks: I understand that driving and the activities associated with the coaching event involve risks, including but not limited to accidents, vehicle damage, and personal injury.

2. Release of Liability: I hereby release, waive, discharge, and promise not to sue [Event Organizer/Company Name], their officers, employees, agents, representatives, and volunteers (hereinafter referred to as "Releasees") from any and all claims, liabilities, damages, losses, or expenses arising from my participation in this event.

3. Indemnification: I agree to indemnify and hold harmless the Releasees from any and all claims, actions, damages, or liabilities arising out of my participation in the event, whether caused by my own negligence or otherwise.

4. Medical Treatment: I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, or illness during the event. I understand that I am responsible for any medical costs incurred.

5. Governing Law: This waiver and release shall be governed by and construed in accordance with the laws of the State of [Your State].

6. Parent/Legal Guardian Consent (if participant is under 18 years of age): I, the undersigned parent or legal guardian of the minor participant named below, acknowledge that I have read this waiver and hold harmless agreement. Driving services offered by Concierge Driving Coach may be be counted as observation hours in accordance with the Massachusetts RoadReady Permit Driver Requirement
